1. สำเร็จการศึกษาระดับทันตแพทยศาสตรบัณฑิต จากสถาบันอุดมศึกษาที่กระทรวงการอุดมศึกษา วิทยาศาสตร์ วิจัยและนวัตกรรมรับรอง 2. ได้แต้มระดับคะแนนเฉลี่ยสะสมไม่ต่ำกว่า 2.50 3. มีผลคะแนนภาษาอังกฤษผ่านตามเกณฑ์ของบัณฑิตวิทยาลัย มหาวิทยาลัยมหิดล 4. ขึ้นทะเบียนและได้รับอนุญาตให้เป็นผู้ประกอบวิชาชีพทันตกรรม 5. มีประสบการณ์ในการประกอบวิชาชีพทันตกรรมอย่างน้อย 1 ปี 6. ผู้ที่มีคุณสมบัติต่างจากที่กําหนดใน 1 ถึง 5 อาจได้รับการพิจารณาให้สมัครเข้าศึกษาได้ โดยให้อยู่ในดุลยพินิจของคณะกรรมการบริหารหลักสูตร และคณบดีบัณฑิตวิทยาลัย
